FOR ASSISTANCE 

As a family seeking financial assistance from the Care Fund for rent or mortgage purposes, we want to emphasize that our primary goal is to provide support during your child's health crisis. We genuinely aim to provide assistance and alleviate the financial burden you face, allowing you to focus on your child's well-being and care.

Though we can’t fix the severity of your situation, what we can offer is some peace of mind while your rent or mortgage payments are made on your behalf, should you be approved for assistance.

ELIGIBILITY
REQUIREMENTS

There is specific criteria that must be met to be eligible to receive available financial assistance.

APPLICATION
PROCESS

Eligible families can complete the 3-step application below. For approved applicants, Care Fund does not expect repayment. Payment is sent directly to the mortgage lender or landlord.

Care Fund is unable to assist with past delinquent rent/mortgage payments or pre-foreclosures. Families must be current on payments for consideration of grant request.

3-STEP PROCESS

Please accept our help by following this 3-step application process and asking questions along the way by emailing info@thecarefund.org. You may fill out the application and medical certification online, (applications provided through our online application may be processed faster for review) or download the forms to be emailed to info@thecarefund.org.

STEP #1

COMPLETE THE APPLICATION

Complete all questions in the application. If a question does not pertain to your situation, simply write ‘N/A’ in the field. During this step you will be asked to provide two of your most recent pay stubs from all income sources and all pages of your most recent mortgage statement or lease agreement and scan with your application.

Online application are prefered for faster processing and review. 

PLEASE NOTE:

We will contact you if information appears to be missing, as it may impact or delay the decision-making process by Care Fund’s Review Committee.

STEP #2

HAVE YOUR PROVIDER COMPLETE THE MEDICAL CERTIFICATION

All fields of the medical certification must be submitted by your child’s medical provider or hospital social worker. Medical certifications cannot be accepted directly from family. The form can be completed online or downloaded to be emailed to info@thecarefund.org.  

STEP #3

WAIT FOR CARE FUND’S APPLICATION REVIEW AND CONTACT

The final step is Care Fund’s time to review and verify the information you and your medical team have provided, and to discern your application. Follow-up questions may be done verbally or through email correspondence. Application approval is made at the sole discretion of Care Fund on a case-by-case basis, based on available funds.

THIS STEP INCLUDES:

  • Verification of eligibility based on Care Fund giving guidelines.

  • Verification of information (employment, income, mortgage/rent (current status or no pre-closure, medical certification, etc.).

  • A meeting of Care Fund’s Review Committee to determine whether assistance will be approved or denied.

  • Contact by a member of our team to inform you of the Review Committee’s final determination. If you are awarded assistance, we will review the parameters and timeframe with you at this time.
